Why Grip Strength Predicts Longevity Better Than Cardio

Dr. Darryl Leong's team at the Population Health Research Institute at McMaster University published the PURE study in The Lancet in 2015 — tracking 140,000 adults across 17 countries. Every 5 kg decrease in grip strength was associated with a 17% increase in cardiovascular mortality and a 16% increase in all-cause mortality. Grip strength outperformed systolic blood pressure as a predictor of death.

That finding has been replicated multiple times. Dr. Setor Kunutsor at the University of Bristol led a 2022 BMJ meta-analysis of 1.9 million participants that confirmed the association: low grip strength independently predicts heart disease, stroke, respiratory disease, cancer mortality, and all-cause death. The relationship held after adjusting for age, sex, BMI, and physical activity level.

Why grip? Researchers believe grip strength is a biomarker for overall muscle quality and neuromuscular integrity. It reflects systemic factors — hormonal status, inflammation, nutritional adequacy, nervous system function — that individual fitness tests cannot capture.

The practical threshold appears to be around 35 kg for men and 20 kg for women, measured with a Jamar hand dynamometer. Below these values, mortality risk increases sharply. Above them, the protective benefit continues to increase but with diminishing returns.

Training grip is straightforward. Dead hangs from a pull-up bar: work up to 60 seconds. Farmer's carries with heavy dumbbells or kettlebells: three sets of 40-meter walks. Plate pinches: hold two 10-pound plates smooth-side-out for time. Twice per week is sufficient.

Dr. Richard Bohannon, a physical therapist and professor emeritus at the University of Connecticut, published normative grip strength data in the Journal of Hand Therapy (2019, n=12,400) stratified by age and sex. His reference values show that grip strength peaks at ages 30-34 (average 54 kg for men, 33 kg for women) and declines by roughly 1-2% per year after 50. The practical implication: if you are over 40 and not specifically training grip, you are almost certainly losing it — and the research says that loss correlates with broader health decline more reliably than any blood test or imaging study.

You do not need a specialized grip training program. If your training already includes deadlifts, pull-ups, rows, and carries performed without straps, your grip is likely adequate. The people at risk are those who avoid these movements — or who rely on machines that eliminate grip demand entirely.
