VO2 Max: What It Means and How to Improve It

The Norwegian University of Science and Technology's HUNT Fitness Study — tracking 4,631 healthy adults over 15 years — found that VO2 max measures the maximum volume of oxygen your body can consume during intense exercise, expressed in milliliters per kilogram of body weight per minute (mL/kg/min). It is the gold standard for cardiovascular fitness and one of the strongest predictors of all-cause mortality.

Population averages: untrained adults score 30-40 mL/kg/min. Recreational athletes score 40-55. Competitive endurance athletes score 55-70. Elite endurance athletes score 70-85. The highest recorded VO2 max belongs to a cross-country skier at 97.5 mL/kg/min.

Your VO2 max is partially genetic — estimated at 50% heritable. But the trainable component is substantial. An untrained individual can improve VO2 max by 15-25% with consistent aerobic training. Even highly trained athletes can add 3-5% with optimized programming.

The most effective training protocol for improving VO2 max is 4x4 interval training: four minutes at 90-95% of maximum heart rate, followed by three minutes of active recovery, repeated four times. Two sessions per week of this protocol improve VO2 max by 5-8% in 8-10 weeks, according to research from the Norwegian University of Science and Technology.

The longevity connection is striking. A 2018 study in JAMA Network Open followed 122,000 adults for 23 years and found that moving from the bottom 25th percentile of fitness to above average reduced all-cause mortality by 50%. Reaching elite fitness levels reduced mortality by 80%. No pharmaceutical intervention achieves this.

You do not need a lab test to estimate your VO2 max. Many fitness watches estimate it from heart rate and pace data. For a rough self-assessment, time yourself on a 1.5-mile run. Under 10 minutes suggests a VO2 max above 45 mL/kg/min. Under 8 minutes suggests above 55. Dr. Peter Attia, a physician focused on longevity medicine, considers VO2 max the single most powerful metric for predicting healthspan — more predictive than cholesterol, blood pressure, or blood glucose levels.
